Conditions of Employment

• Are you a citizen or do you have a visa which permits you to work in the United States?
• Are you physically able to perform the duties of the job for which you are applying?

Successful WSP and FBI background clearance check; 2. Washington State Sexual Misconduct Disclosure Release Form; 3. I-9 Employment Eligibility (citizen or legal alien status); 4. W-4 Form; 5. Washington State Department of Retirement Status Form; 6. Immunization History (for persons born January 1, 1957 or after) - Completion of District Form to be placed on file with the District Nurse; 7. Online mandatory trainings per Lynden School District policy; 8. Hepatitis B Training (if required); 9. Social Security Card and valid Driver's License (for identification upon hiring), to be copied by Personnel Office. BY SELECTING "YES", YOU AGREE TO PROVIDE /COMPLETE THESE REQUIRED ITEMS.
• I authorize Lynden School District 504 to make an investigation of my personal, educational, vocational, or employment history. I further authorize any current or former employer, firm, corporation, educational or vocational institution or government agency to provide Lynden School District 504 with any information they have regarding me. I hereby release and discharge Lynden School District 504 and all those who provide information from any and all liability as a result of requesting, furnishing and receiving this information.

• Do you have, or are you willing to obtain, a valid Washington State driver's license and Class B, CDL with appropriate endorsements?
• Do you have, or are you willing to obtain, a valid school bus driver authorization and five-year complete school bus driver abstract?
• Do you have, or are you willing to obtain, valid first aid qualification (eight-hour Red Cross or equivalent)?
• Are you able to successfully meet all the requirements of WAC Chapter 392-144 (drug, disclosure, fingerprinting)?
